What is a Drug Formulary?
A drug formulary is simply a list of the most appropriate and cost-effective medications from which participating physicians prescribe. Hospitals have been using formularies since the 1960s, and most health plans now use them as well. You will note on the formulary that a drug’s co-payment is assigned to a tier and that in some cases both the brand name and generic are listed.
Prescription plan information is included in both the Benefit Grid and the SPD.
