The NY44 Health Benefits Plan Trust has a unique opportunity to provide resources and information to engage enrollees and their families to become well-informed patients and consumers of health care services. We believe one of the best ways to achieve good health and reduce health care costs is to empower individuals to take personal responsibility for their health.

The NY44 Living Healthy Program offers rewards and reimbursements based on four key principles:

Prevention: Routine preventive screenings can help detect risk factors for disease or early detection of existing diseases.
Education: Using self-assessment tools, such as the Independent Health Risk Assessment, can help with understanding current health status and what changes need to be made.
Fitness: Incorporating physical activities into a daily routine can reduce illness and the onset of chronic disease.
Healthy Choices: Lifestyle choices can have a positive effect on your health.

	Physical Exam	Fitness Activity	Smoking Cessation
Description	Effective July 1, 2011, get paid $25 for having a physical exam from your primary care physician. This reward comes in the form of a one-time payment, per eligible member. (Up to a maximum of two per year for family plans)	This reimbursement program rewards individuals for engaging in sustained, on-going physical fitness programs or purchasing fitness equipment for home. The Trust also rewards districts that provide education to their staff through seminars, wellness fairs or printed materials.	Smoking cessation prescription drugs have been added to the drug formulary. This will make it easier for enrollees to enter and continue their participation in an approved smoking cessation program.

Plan Year July 1, 2011 - June 30, 2012	Exams performed July 1, 2011 - June 30, 2012 are eligible for the reward this plan year.	All receipts are dated between July 1, 2011 and June 30, 2012.

District Initiatives		Up to $1,000 for approved wellness activities. Offered to districts where the NY44 plan is the only option offered within the district.
