The plan features $0 co-payment for the visits you have more frequently – office visits, children’s well visits, X-rays, – to those you have less frequently – outpatient surgery and maternity visits.

Prescription Plan.

The prescription plan includes a $0 co-payment for Tier 1 generic prescriptions, Tier 2 inhalers, Tier 1 & 2 diabetic medications, diabetic supplies such as insulin needles, syringes, lancets, test strips.

Vision Plan.

A vision plan is offered with our medical plan that provides a cost savings to you.

| Wellness Reimbursements. |
The Trust rewards individuals for engaging in sustained, on-going physical fitness programs or purchasing fitness equipment for home. The Trust also rewards districts that provide education to their staff through seminars, wellness fairs or printed materials. Specific guidelines apply to both reimbursements. | Dental Pay Plus Plan. |
A dental plan that features preventative, restorative and orthodontia services is offered through ProBenefits Administrators. Like most plans, using the in-network dental professionals will provide you with cost savings. Note: not every district participates in the dental plan. |
Learning More About Your Benefits
There are two ways for enrollees to learn more about the benefits offered.

The Benefit Summary Grids are formatted for you to quickly determine copayments and what services are included in-network and out-of-network. There are two grids: the Medical & Vision grid and the Dental grid.

The Summary Plan Description is the legal document outlining the complete plan. It is referred to as the SPD. There are two SPDs: the Medical SPD and the Dental SPD.
