The Trust rewards individuals for engaging in sustained, on-going physical fitness programs or purchasing fitness equipment for home. It is our belief that providing these incentives can motivate our plan participants to adopt leisure activities that promote a healthier lifestyle.
The Trust also rewards districts that provide education to their staff through seminars, wellness fairs or printed materials. School Districts are eligible if 100 percent of the School or District is part of the Trust. Approved activities or program materials may include wellness fairs; informational Health and Wellness magazines, DVD or videos; educational seminars; or health lifestyle speakers.
For single and family plan participants, we offer reimbursements for a variety of equipment and sustained health and wellness activities. On a yearly basis, here are the reimbursement caps:
Single Plan: Up to $100 reimbursed for approved activities or equipment.
Family Plan: Up to $150 reimbursed for approved activities or equipment.
District Plan: Up to $1,000 reimbursed for approved activities.
2009-10
You have until August 31, 2010 to submit receipts for the plan year running from July 1, 2009 to June 30, 2010.
2010-11
Receipts must be dated between July 1, 2010 - June 30, 2011
